Registration for the Winter 2005 season is now closed.

Waiver/Release Form UPDATED 09/2004 - This is the new waiver form mandated by StorageTek. ALL PLAYERS MUST SUBMIT SIGNED FORMS TO THE WELLNESS CENTER BY AUGUST 19 - NO EXCEPTIONS!

Emergency Contact Form - This form needs to be filled out prior to the beginning of EVERY season. Either turn it in to the Wellness Center when you pay your fees or bring it with you to your first game.

• Two-Day Players will be charged two times the regular fee.
• Goalies will receive 1/2 off their fees for the division in which they are playing in goal.
• Captains will receive 1/2 off their fees for the division in which they are captaining.

Payment must be submitted directly to the front desk in the Wellness Center prior to the beginning of the season along with the above waiver form. All other payment forms and methods are obsolete.
