Prime Wheels Order Form

Ben Fisketjon, our league Wellness Center contact, now handles all Prime Wheels wheel and bearing orders for the league. To order wheels or bearings at the discounted rates available to the league, you will need to:

1. download the form or pick one up at the Wellness Center front desk,
2. fill it out, and
3. return it to the Wellness Center along with payment.

Once Ben gets at least ten orders together, he will take them up to Prime Wheels. Once the order is in he will e-mail everyone and you can pick them up at the Wellness Center front desk. Contact Ben at fiskeba@louisville.stortek.com or (303) 661-6740 if you have any questions.
